For example, Provider Online Services permits current Providers to check Authorization Requests Status, Billing and Payment history and Past/Previous Request history.

You may access Fax Authorization Request Forms, General Information, Company services, agreement documents; contact our customer services center, and much more. During registration, a Provider member must provide personally identifiable information, including name, Provider identification number, Full Name and email address as well as the patients name, Date of Birth, Procedure information, Date Of Service, type of service. This information allows us to verify and authenticate the member and establish a secure online services account as well as provide accurate authorization requests.

Once you register for our online services sites, you may be asked for additional personally identifiable information to allow you to gain access to additional online application tools, payment options or other services.

As part of the Provider Online Services registration process, users may provide their e-mail address or membership fax request must be sent by fax request. Providers may choose to complete registration and obtain their login info by e-mail, and they may choose to receive e-mail alerts regarding account changes, billing, Fax Authorization Status and payment information. Providers who do not want to receive e-mails about our future “Recommended” services may out-opt of e-mail transmissions by informing customer service by fax or emailing the decision to “opt-out” of future offers we may send.. providers  may be required to provide their email address so that MDInsurance  may send communications to the user about their online services account and/or other business-related communications.
